D16 gate contract (per step)
Every gated step exposes: confidence(input) -> float, gate(input, threshold) -> (accept|escalate, score), deterministic_entry(input) (the accept-path result), plus MODE_FIELD (the provenance field
naming which mode produced the output). See Explanation.
